A professional person who is specifically trained, competent, and performs assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of the registered nurse (RN).
Job Details
As a Home Health Aide, you will provide essential hands-on care and support to patients in their homes. Your role is critical in ensuring the comfort and well-being of those you serve while working under the supervision of a registered nurse.
Qualifications
- Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
- Have a sympathetic attitude toward the care of the sick and elderly.
- Ability to carry out directions, read, and write.
- Maturity and ability to deal effectively with the demands of the job.
Responsibilities
- Understand and adhere to established Agency policies and procedures.
- Perform personal care, bathing, and hands-on care as assigned.
- Complete visit records in a timely manner as per Agency policy.
- Report changes in the patient’s condition and needs to the RN.
- Perform household services essential to health care in the home as assigned.
- Assist with ambulation and exercises as assigned.
- Perform simple procedures as an extension of therapy or nursing services, e.g., range of motion (ROM) exercises as assigned.
- Assist with medications that are ordinarily self-administered as assigned.
- Attend in-service and continuing education programs as scheduled.
- Participate in patient care conferences as scheduled.
- Works indoors in Agency offices and patient homes, traveling to/from patient homes.
